THE POSITION

Obesity is a chronic, biologically regulated disease — not a lack of willpower.

The body regulates weight through the brain, gut and fat-tissue hormones, energy expenditure, genetics, sleep, medications, environment and behavior. That is why “eat less, move more” is biologically true and clinically incomplete. We do not swing to the opposite extreme either: nutrition, activity and behavior matter. Weight is multi-causal, and treatment should match what is actually driving it.

Don’t treat the scale. Treat the person.

Understand

Weight history, medications, sleep, eating patterns and goals — before any prescription. A prescription is not a diagnosis.

Test

What actually matters: adiposity, metabolic health, cardiovascular risk, and the liver — including fibrosis, not just fat.

Treat

To a real goal, not a scale number: diabetes remission, cardiovascular risk, fatty liver, sleep apnea, mobility, quality of life.

Sustain

Protect muscle during loss and plan maintenance before starting. Regain is biology; we plan for it.

WHY WE ANSWER THIS WAY
A lower number on the scale is not automatically better health.

It would be easier to sell one diet, one drug, or one supplement — or to tell everyone their weight is simply their fault. Neither is honest. We take the biology seriously, look at what the evidence actually says, define what “better” means for you, and measure whether treatment achieved it. You deserve clinicians who will say “there’s no good evidence for that yet” and mean it — and who will also say “this one is worth trying, and here’s how we’ll know.”

THE DIFFERENTIATOR

We don’t just tell you that you have fatty liver. We ask what it means.

Fat in the liver is common. Scarring — fibrosis — is what changes your future, and the two are not the same. Our metabolic-liver approach separates finding fat from finding risk: steatosis assessment, metabolic risk, FIB-4, and elastography when it is actually indicated.
